This support is delivered by qualified nurses in the participant’s home or community setting to help them manage their health conditions and maintain their independence. With personal nursing support, participants with complex health needs, including wound care, medication management, catheter care, and monitoring chronic conditions to maintain health and well-being
at home or in the community.
Key Services Provided by Community Nursing Care:
1. Wound Care Management:
* Providing treatment for minor and complex wounds, including cleaning, dressing changes, and monitoring for signs of infection or healing progress.
2. Medication Management:
* Assisting with administering medication, monitoring its effects, and ensuring that participants are taking the correct doses at the right times.
* This may include administering injections, IV medications, or other forms of medication that require professional support.
3. Chronic Disease Management:
* Helping participants manage chronic health conditions such as diabetes, respiratory diseases, heart disease, or other long-term illnesses. Nurses monitor symptoms, assist with treatments, and educate participants on managing their conditions.
4. Continence Support:
* Providing assistance with continence management, including catheter care, stoma care, or helping participants use continence aids effectively.
5. Monitoring Vital Signs:
* Regularly checking participant’s; blood pressure, heart rate, blood sugar levels, and other vital signs to ensure they remain stable and identify any potential health issues early.
6. Post-Hospital Care:
* Supporting participants after being discharged from the hospital, including post-operative care, rehabilitation support, and helping them transition back into their home environment.
7. Education and Health Promotion:
* Educating participants and their support workers on how to manage medical conditions, proper use of medical equipment, and promoting healthy lifestyles to prevent complications.
8. Specialised Nursing Procedures:
* Performing more complex nursing procedures like catheter changes, PEG feeding (tube feeding), tracheostomy care, or suctioning for participants who require high-level nursing interventions.
9. Palliative Care Support:
* Providing end-of-life care to ensure participants remain comfortable and supported in their own home during palliative care stages.
